We offer Home Health Services including Intravenous Injection Therapy, Respiratory Therapy, Registered Nurses, Licensed Vocational Nurses and Physical Therapists. We have a professional team that will meet your needs in a timely fashion.

What home health covers
Disciplines are assigned from the plan of care, not sold as a package.
Skilled nursing
Wound care, medication management and education, injections, vital signs, and the assessments the plan of care calls for.
Home health aide support
Help with bathing, dressing, mobility and the daily routine, scheduled around the household.
Physical and occupational therapy
Mobility, strength, balance and daily-living work carried out in the patient's own home.
Speech therapy
Support for communication, cognition, voice and swallowing needs, within the clinician's scope.
Medical social services
Help navigating resources, planning and the practical problems that sit around a clinical plan.
Coordination with transportation
When the patient also needs rides to appointments, the same organization arranges them.

Plan of care
Disciplines, visit frequency and goals are set with the ordering clinician.
Visits and communication
Care is delivered on schedule, changes are escalated, and progress goes back to the ordering clinician.

Do we need a clinician's order?
Skilled home health is provided under an order. If you do not have one yet, contact us and we will walk you through what is needed and who has to sign it.
How often are visits?
Visit frequency comes from the plan of care rather than a fixed package, and it is adjusted as the patient improves or their needs change.
Can home health and therapy be combined?
Yes, and it is one of the main reasons to take both from one provider — nursing and therapy visits can be scheduled around each other instead of colliding.
What does the caregiver have to do?
That depends on the plan. We train caregivers deliberately, because the days between visits are where most of the recovery happens.
